Luận án Thực trạng và hiệu quả cải thiện dịch vụ chăm sóc sơ sinh tại cơ sở y tế một số huyện thuộc tỉnh Thanh Hoá. Theo đánh giá của Liên hiệp quốc trong năm 2015, Việt Nam là một trong những quốc gia đã đạt chỉ số giảm tử vong trẻ dưới 1 tuổi của Mục tiêu Thiên niên kỷ, trong đó tử vong sơ sinh đóng góp quan trọng vào chỉ tiêu này [1]. Tỷ lệ tử vong trẻ em đã giảm đáng kể từ 58/1000 năm 1990 xuống còn12/1000 trẻ đẻ năm 2014[2]. Tuy nhiên để duy trì thành quả đó, Việt Nam cũng phải vượt qua 2 thách thức lớn, đó là còn sự khác biệt về sức khoẻ trẻ em giữa các vùng miền và tỷ lệ tử vong sơ sinh cao, đặc biệt là trong tuần đầu sau đẻ.
Chăm sóc sơ sinh đã được Bộ Y tế quan tâm đặc biệt trong thập kỷ qua, tuy nhiên mức độ giảm tử vong trẻ sơ sinh vẫn còn chậm hơn nhiều so với tử vong ở trẻ dưới 1 tuổi và tử vong ở trẻ dưới 5 tuổi[3]. Theo báo cáo của Vụ Sức khoẻBà mẹ và Trẻ em năm 2014, tỷ lệ tử vong sơ sinh đang chiếm khoảng 60% số tử vong trẻ dưới 5 tuổi và hơn 70% tử vong ở trẻ dưới 1 tuổi. Vì vậy các can thiệp giảm tử vong sơ sinh vẫn cần được đặt ở vị trí ưu tiên hàng đầu trong các can thiệp về cứu sống trẻ em[4].
Nguyên nhân chínhgây tử vong ở trẻ sơ sinh là đẻ non, nhiễm khuẩn và ngạt đều là các nguyên nhân có thể phòngtránh được. Tiếp cận chăm sóc liên tục từ gia đình, cộng đồng đến cơ sở y tế với các can thiệp như tiêm vắc xin phòng uốn ván, thực hiện cuộc đẻ an toàn, hồi sức sơ sinh, nuôi con bằng sữa mẹ, phòng chống viêm phổi sơ sinh v.v.. đã được chứng minh là có thể giảm tới 75% tỷ lệ tử vong sơ sinh[5]. Vì thế WHO đang kêu gọi tiến hànhthêm nhiều nghiên cứu nhằm cung cấp các bằng chứng rõ rệt hơn về hiệu quả của can thiệp giảm tử vong sơ sinh cũng như duy trìtính hiệu quả bền vững của các mô hình can thiệp, đặc biệt cho các nước thu nhập thấp và trung bình, nơi chiếm tới 98% số tử vong sơ sinh trên toàn thế giới[6].
Quyết địnhPhê duyệt “Hướng dẫn tổ chức thực hiện đơn nguyên sơ sinh và góc sơ sinh tại các tuyến y tế” của Bộ Y tế ban hành năm 2011 là một văn bản pháp lý quan trọng nhằm cụ thể hóa Chỉ thị 04/BYT-CT về “tăng cường chất lượng chăm sóc và giảm tử vong sơ sinh” trong toàn quốc[7]. Tuy nhiên, do hạn chế về nguồn nhân lực, cơ sở hạ tầng, trang thiết bị và thuốc thiết yếu cho chăm sóc sơ sinh (CSSS), việc thành lập và vận hành đơn nguyên sơ sinh ở bệnh viện huyện và góc sơ sinh ở trạm y tế (TYT) xã chưa được thực hiện ở tất cả các cơ sở y tế trong toàn quốc[8]. Đồng thờicác nghiên cứuđánh giá một số dịch vụ và hiệu quả triển khai các can thiệp về chăm sóc sơ sinh còn rất khó khăn và hiện có số lượng hạn chế. Tại tỉnh Thanh Hoá,cho đến nay vẫn chưa có nghiên cứu về cung cấp dịch vụ CSSS tại tuyến huyện và tuyến xã. Chính vì những lý do trên, nghiên cứu“Thực trạng và hiệu quả cải thiện dịch vụ chăm sóc sơ sinh tại cơ sở y tế một số huyện thuộc tỉnh Thanh Hoá”được thực hiện nhằm các mục tiêu sau:
1.Mô tả thực trạng chăm sóc sơ sinhvà một số yếu tố liên quan tại 4 huyện tỉnh Thanh Hóa năm 2015.
2. Đánh giá hiệu quả một số giải phápcan thiệp nhằm cải thiện cung cấp dịch vụ chăm sóc sơ sinh tại 4 huyệntrên năm 2015-2016.
